Suplavat® Tablets (Amlodipine 5 mg / Telmisartan 40 mg)

Suplavat

Category: CARDIOVASCULAR

Product Description

Suplavat® is a fixed-dose combination of Amlodipine 5 mg, a long-acting calcium channel blocker, and Telmisartan 40 mg, an angiotensin II receptor blocker (ARB). This dual-action antihypertensive therapy provides effective and sustained blood pressure control through complementary mechanisms of action.

Amlodipine relaxes vascular smooth muscle to reduce peripheral vascular resistance, while Telmisartan blocks the vasoconstrictive effects of angiotensin II, promoting vasodilation and reducing aldosterone secretion. Together, they provide superior blood pressure reduction compared with either agent alone and help reduce the risk of cardiovascular complications associated with hypertension.

Suplavat® is suitable for patients whose blood pressure is not adequately controlled with monotherapy or those requiring combination therapy as initial treatment.

Indications

Suplavat® is indicated for:

  • Treatment of essential hypertension in adults.
  • Replacement therapy in patients whose blood pressure is adequately controlled with amlodipine and telmisartan administered separately.
  • Initial combination therapy in patients likely to require more than one antihypertensive agent to achieve blood pressure targets.
  • Reduction of cardiovascular risk associated with uncontrolled hypertension as part of comprehensive cardiovascular management.

Contraindications

Suplavat® is contraindicated in patients with:

  • Hypersensitivity to amlodipine, telmisartan, dihydropyridine derivatives, or any component of the formulation.
  • Pregnancy, particularly during the second and third trimesters.
  • Severe hepatic impairment or biliary obstructive disorders.
  • Severe hypotension or cardiogenic shock.
  • Concomitant use with aliskiren in patients with diabetes mellitus or severe renal impairment (eGFR <60 mL/min/1.73 m²).
  • Known hereditary fructose intolerance (where applicable to the formulation’s excipients).

Use with caution in patients with renal impairment, heart failure, aortic or mitral valve stenosis, hypertrophic obstructive cardiomyopathy, or volume depletion.

Dosage and Administration

Suplavat® should be taken orally once daily, with or without food, preferably at the same time each day.

Adults

  • Recommended dose: One tablet (Amlodipine 5 mg / Telmisartan 40 mg) once daily.
  • Dosage should be individualized based on the patient’s blood pressure response and clinical condition.
  • Patients not adequately controlled on either amlodipine or telmisartan alone may be switched to Suplavat®.
  • If additional blood pressure reduction is required, titration to a higher-strength combination (where available) or the addition of another antihypertensive agent may be considered.

Elderly Patients

  • No routine dose adjustment is required, although careful monitoring is recommended, particularly when initiating therapy.

Patients with Renal Impairment

  • No dosage adjustment is generally required in mild to moderate renal impairment.
  • Monitor renal function and serum potassium periodically.

Patients with Hepatic Impairment

  • Use with caution in mild to moderate hepatic impairment.
  • Contraindicated in severe hepatic impairment or biliary obstructive disorders.
