Category: ANTI DIABETICS
Product Description
Suphage® is a fixed-dose combination of Metformin Hydrochloride and Glimepiride, two complementary oral antidiabetic agents that provide effective glycaemic control in patients with Type 2 Diabetes Mellitus.
Metformin reduces hepatic glucose production, decreases intestinal glucose absorption, and improves insulin sensitivity, while Glimepiride stimulates insulin secretion from pancreatic β-cells. Together, they help achieve sustained blood glucose control, reduce HbA1c levels, and lower the risk of diabetes-related complications when combined with a healthy diet and regular exercise.
Available Strengths
- Suphage® 500/2 – Metformin Hydrochloride 500 mg + Glimepiride 2 mg
- Suphage® 1000/1 – Metformin Hydrochloride 1000 mg + Glimepiride 1 mg
- Suphage® 1000/2 – Metformin Hydrochloride 1000 mg + Glimepiride 2 mg
Indications
Suphage® is indicated as an adjunct to diet, exercise, and lifestyle modification for:
- Treatment of Type 2 Diabetes Mellitus in adults whose blood glucose is inadequately controlled with metformin or glimepiride monotherapy.
- Replacement therapy in patients already stabilized on metformin and glimepiride administered as separate tablets.
- Improvement of glycaemic control and reduction of HbA1c in patients requiring combination oral antidiabetic therapy.
- Long-term management of Type 2 Diabetes Mellitus to reduce the risk of microvascular complications associated with poor glycaemic control.
Contraindications
Suphage® is contraindicated in patients with:
- Hypersensitivity to metformin, glimepiride, other sulfonylureas, sulfonamides, or any component of the formulation.
- Type 1 Diabetes Mellitus.
- Diabetic ketoacidosis, diabetic pre-coma, or diabetic coma.
- Severe renal impairment (eGFR <30 mL/min/1.73 m²).
- Severe hepatic impairment.
- Acute or chronic metabolic acidosis, including lactic acidosis.
- Conditions associated with tissue hypoxia such as severe heart failure, respiratory failure, recent myocardial infarction, or shock.
- Acute conditions that may impair renal function, including severe dehydration or severe infection.
- Excessive alcohol intake or acute alcohol intoxication.
- Pregnancy and breastfeeding unless clearly indicated by the treating physician.
Dosage and Administration
Suphage® should be taken orally with meals to reduce gastrointestinal side effects. Dosage should be individualized according to the patient’s glycaemic response and tolerability.
Adults
- Initial dose: One tablet once daily with breakfast or the first main meal.
- The dose may be adjusted gradually based on blood glucose and HbA1c levels.
- Patients switching from separate metformin and glimepiride tablets may be initiated on the equivalent Suphage® strength.
- The maximum recommended daily dose should not exceed:
- Metformin: 2,000 mg/day
- Glimepiride: 8 mg/day
Regular monitoring of blood glucose, HbA1c, and renal function is recommended throughout treatment. Elderly patients and those with renal impairment should receive the lowest effective dose and be monitored closely because of the increased risk of hypoglycaemia and lactic acidosis.
